Schoolcraft College will be using digital forms to collect employee vaccination information and COVID-19 testing results as required by the Occupational Safety and Health Administration (OSHA). Please understand that the status and details of these requirements are still evolving. Any changes to this process will be communicated via this web page.
Complete this form with information about your COVID-19 vaccination(s). Uploading a digital copy of your vaccination card can be done with a smartphone camera.
If you haven't started the vaccination process yet, do not submit this form.
Need to make an update? You may submit this form multiple times to provide additional vaccine information (i.e. after receiving a second vaccination or booster). All submissions will be reviewed, and you may receive an email asking you to re-submit if there are any issues with your upload.
